Student Medication and Documentation

 
If your student has a food allergy, asthma, diabetes, seizure disorder, or other medical condition that requires medication to be kept at school, please fill out the proper documentation and drop off the medicine at the school's front desk.

 a. Medication at School Authorization Form  b. The Action Plan that meets your student's needs
 
Important Reminders: 
All medical documentation is good for one school year at a time, it does not carry over to the next school year.
 
All student medication will need to be picked up at the end of the school year. 
 
Student medication that is not picked up will be discarded, as we cannot safely store it over the Summer break.
